We are asking that each team designate a TEAM LEADER. This leader is responsible for the following:

Submitting payment for ALL riders on your team when submitting this form. Your entry is not confirmed until payment for ALL riders has been received.
Confirming that all riders on your team have completed the current Myopia Hunt Online Release of Liability.
